Agreement to Terms
By accessing or using the TeleTonicMD website (www.teletonicmd.com) or receiving our virtual primary care services, you ("Patient," "you," or "your") agree to be bound by these Terms of Service ("Terms"). If you do not agree to these Terms, you may not use our services or website.
About TeleTonicMD
TeleTonicMD provides virtual direct primary care services and in-home medical consultations in Illinois, with a focus on the Chicagoland area. Our services are provided by Dr. Azka Afzal, a board-certified physician licensed to practice medicine in Illinois.
Monthly Direct Primary Care Membership: TeleTonicMD offers a membership at $200/month for direct, ongoing access to Dr. Afzal — including secure messaging you can send anytime for follow-up questions, support between visits, and priority scheduling, without insurance hassles or restrictions. Consultation visits are billed separately at the flat rates listed under “Payment Terms.” This approach provides predictable, transparent healthcare costs and greater accessibility to primary care.

Virtual Primary Care

Patient Eligibility and Requirements
Geographic Limitations
- Telehealth services: Available only to patients physically located in Illinois at the time of each visit
- In-home visits: Limited to the Chicagoland area
Age Requirements
- Services available for patients 18 years and older
- Minors require parent/guardian consent and presence during consultations
Technical Requirements
- Reliable internet connection for video consultations
- Compatible device (computer, tablet, or smartphone)
- Updated web browser or mobile app
- Quiet, private location for consultations
Appointment Booking and Cancellation
Scheduling
- Appointments must be scheduled in advance
- Same-day appointments subject to availability
- Confirmation required 24 hours before appointment
Cancellation Policy
- Patient Cancellations: Must cancel at least 24 hours in advance
- Late Cancellation Fee: $25 for cancellations with less than 24 hours’ notice
- No-Show Fee: $50 for missed appointments without notice
- Provider Cancellations: Full refund if we need to cancel
Rescheduling
- Patients may reschedule up to 24 hours before appointment
- Emergency rescheduling considered on case-by-case basis
Payment Terms
Fee Structure
- Monthly Membership: $200/month (Direct Primary Care access; consultations billed separately)
- Initial Consultation: $150 (flat fee)
- Follow-up Visits: $100 (flat fee)
- Payment Plans: Available based on income verification
- In-Home Visits: Priced individually

Payment Timing

- Disputed charges handled within 30 days of service
Medicare and Membership
Membership fees are not insurance and are not billed to Medicare. Certain eligible clinical services may be billed to Medicare separately; membership benefits are provided independently of any insurance or Medicare coverage.
Telehealth-Specific Terms
Telehealth Consent
By scheduling or participating in a telehealth visit, you consent to receive care via secure video and messaging technologies, and you acknowledge the limitations of telehealth described in these Terms. You confirm that you are physically located in Illinois at the time of each telehealth visit and agree to disclose your physical location truthfully before each visit. Telehealth may not be appropriate for all conditions, and Dr. Afzal may determine that in-person care is required.
Technology Platform
- We use HIPAA-compliant video conferencing platforms
- Technical support available during business hours
- Patient responsible for their device and internet connection
Consultation Environment
- Patient must be in a private, secure location
- Background noise should be minimized
- Adequate lighting required for visual examination
- Family members may participate with patient consent
Limitations of Telehealth
- Not suitable for all medical conditions
- Physical examination capabilities are limited
- Laboratory tests require separate arrangements
- Some conditions require in-person evaluation
- Call 911 for life-threatening situations
- Go to nearest emergency room for urgent care
- We will provide emergency contact information during consultations
Medical Practice Standards
Physician-Patient Relationship
- Creating a patient portal account, submitting a form, or otherwise contacting the practice does not, by itself, establish a physician-patient relationship
- A physician-patient relationship is established at your first consultation
- The relationship continues through ongoing care
- Either party may terminate the relationship with appropriate notice
Standard of Care
- Services provided according to accepted medical standards
- Treatment recommendations based on information provided by patient
- Accuracy of patient information is crucial for proper care
Prescription Policy
- Prescriptions issued only when medically appropriate
- Controlled substances have special restrictions
- Patient responsible for pharmacy coordination
- Prescription refills require consultation
Medical Records
- Complete medical records maintained for all patients
- Records available upon patient request
- Transfer of records facilitated when requested
- Records retained according to Illinois state law
Patient Responsibilities

Governing Law and Dispute Resolution

Dispute Resolution Process
- Direct Communication: Attempt to resolve issues directly with practice

- Legal Action: Court proceedings as last resort
Modifications and Updates
Changes to Terms

Termination
Patient-Initiated Termination

- We may terminate relationship for cause (non-payment, non-compliance, etc.)
- 30 days written notice provided when possible
- Emergency care information provided during transition
- Medical records available for transfer
Entire Agreement
These Terms, together with our Privacy Policy and HIPAA Notice of Privacy Practices, constitute the entire agreement between you and TeleTonicMD regarding the use of our services. Any conflicting terms in other documents are superseded by these Terms.
Acknowledgment and Acceptance
By using our services, scheduling an appointment, or continuing to access our website, you acknowledge that you have read, understood, and agree to be bound by these Terms of Service. If you do not agree with any part of these Terms, please discontinue use of our services immediately.
