Our monthly membership covers comprehensive, holistic primary care — unlimited visits, direct access to Dr. Afzal, and care coordination — outside of traditional insurance. Additionally, we accept Medicare for eligible clinical visits, which are billed separately. This means Medicare patients can benefit from both the personalized attention of a membership practice and the coverage they've earned. Payment plans are also available based on income.
We specialize in managing multiple chronic conditions common in Medicare patients, including diabetes, hypertension, heart disease, arthritis, autoimmune diseases, mental health conditions, neurodivergence, and pain disorders. Our integrative approach is particularly effective for patients seeking natural alternatives or those who haven't found relief with conventional treatments alone.
Virtual visits are conducted via secure video conferencing. You'll receive a link before your appointment. Dr. Afzal will conduct a thorough consultation, review your health history, and create a personalized treatment plan. Follow-up care includes secure messaging for questions between visits.
Yes, Dr. Afzal is fully licensed to prescribe medications in Illinois, including controlled substances when appropriate. She also holds certification as a Medical Cannabis Certifying Physician and can discuss all treatment options with you.
